Your first visit
Initial consultation form
Upon entering our office, we will welcome you as a member of our practice. We will help you to complete a patient form, which details your health history and information on your current state of health.
Consultation
Next, you will have a consultation with your chiropractor to discuss your health and concerns. This initial visit is designed for us to learn more about you and your expectations to determine how chiropractic care can meet your needs.
Examination
After your consultation, your chiropractor will perform a complete chiropractic examination for which you will be asked to undress to your underwear (you will be offered a gown to wear). Standard neurological, orthopedic, postural, and physical tests will be performed as well as specialist chiropractic testing. Nothing will be undertaken in our clinic without your consent.
